Office Hours – Monday thru Thursday 8:00am - 5:00pm, closed for lunch 12:30pm – 1:30pm, and Friday 8:00am – 12:00pm.

Payment – Co-payment is due at the time of service. Accepted methods of payment include cash, debit card, and credit card (Visa, MasterCard, & Discover). No checks or American Express is accepted.

Walk-ins – To better serve our patients in the most efficient, timely manner, our office is operated primarily on an appointment-only basis. Walk-in patients may be seen however, please be aware that there will be a wait as scheduled patients will be seen first. This includes patients who may arrive after you that have a scheduled appointment.

Late policy –We ask that you call our office if you feel you may be late to your scheduled appointment. Service is not guaranteed if you show up late but failed to contact our office. As a result, you may be asked to reschedule your missed appointment or your status will be changed to a walk-in.

No-show policy – Patients who schedule appointments but fail to show up will be documented as “no-show.” Patients with multiple no-shows may be terminated from the practice. We understand circumstances may arise that may prevent you from coming in, however, we ask that you contact us as soon as you can so that we may schedule other patients that may be needing medical care.
*New patients who no show for their initial appointment may be asked to find another provider.

Prescription Refills – We ask that you call your pharmacy and have them fax to our office the refill request. If a written prescription is required for a controlled substance, you may call the office and we will contact you when your prescription is ready to be picked up. Please allow up to 48 hours to process all refill requests.  Refills will not be processed outside of normal business hours, weekends or holidays when the office is closed. Please plan your refill request accordingly.

Physician Emergency on call policy – When the office is closed, a physician is on-call after hours. The physician is to be contacted when the office is closed for MEDICAL EMERGENCIES ONLY. If you call for an appointment, prescriptions refills, obtain x-ray or lab results, your call may not be returned until the next business day. In the case of a true life threatening emergency, call 911 or seek treatment at the nearest emergency room on weekends and after hours.

Copying of medical records – Patients or insurance companies requesting copies of medical records will be charged a $25 fee for the first 20 pages and 50 cents thereafter per page.

Patient termination policy – Although it is a rare occurrence, a patient may be terminated from the office. Common reasons for termination include, but not limited to, chronic non-compliance with recommended therapy and/or medications, non-payment of services rendered, repeated no-show, and abusive behavior to staff, physicians, visitors, or other patients. Once a patient is terminated from a physician, they will not be able to be established with any other physicians within the office.
